North Carolina Form Guide

Streamlined Sales Tax Form 

  • Rev. 1/07  E-595E
  • Replaces forms E-526, E-558, E-567, E-575, E-580, E-590, E-599
  • Reason for exemption E (Charitable) and F (Religious) are not accepted in NC.

 

Required fields:

  1. State filled in as NC.
  2. Name of purchaser, including street address, city and state.
  3. Purchaser’s tax ID number with state issued or FEIN or State Driver ID, or Foreign Diplomat number. May still be valid if blank, but ID number entered in Part 5 below.
  4. Name of seller.
  5. At least one selection in Part 4: type of business.
  6. At least one selection in Part 5: reason for exemption. ID number must be entered in this section or in item 3 above.
  7. Signature of purchaser.

 

 

Agricultural Exemption NC Form E-598

  • Rev 01-02
  • Retired 2005, use SST form now

 

Required fields:

  1. Vendor/Seller’s Name.
  2. Either in Part 1. Description of items purchased or
    1. Part 1. Description of items purchased.
    2. Part 2 A box must be checked for what is being purchased or description filled in.
  3. Purchaser’s name.
  4. Purchaser’s address.
  5. Signature.
  6. Date.

Manufacturing Exemption NC Form E-575  

  • Retired 2005, use SST form now

 

Required fields:

  1. Vendor/Seller’s name.
  2. Description of items purchased, which should include the PO, contract number or itemized listing of materials purchased.
  3. Purchaser’s name.
  4. Purchaser’s address.
  5. Signature.
  6. Date.

Certificate of Resale NC Form E-590 

  • Retired 2005, use SST form now

 

Required fields:

  1. Vendor/Seller’s name:
  2. Type of business operated by buyer.
  3. Description of items purchased.
  4. Purchaser’s name.
  5. Purchaser’s address.
  6. Purchaser's state issued resale number.
  7. Signature.
  8. Date.
  • Effective January 1, 2005, Form E-595 is the accepted form and older forms dated after 1/1/05 should not be accepted. A vendor who accepted a Form E-590 (prior version of Resale form) is not required to obtain a new form E-595. The older form will remain valid per SST guidelines.
  • NC exempts federal and state government agencies by regulation and they are not required to complete an exemption certificate. They must only show proof of the use by the agency by way of purchase order, letter or use of a federal credit card.
  • Customers with Direct Pay Permits may present a copy of the state issued letter giving them this status and certificate number. The state recommends the use of the SST form to document these transactions.
  • The MTC form is accepted in NC, but it may only be used for resale transactions.
