Streamlined Sales Tax Form 

  • Rev. 3/06-2

 

Required fields:

  1. State filled in as NJ.
  2. Name of purchaser, including street address, city and state.
  3. Purchaser’s tax ID number with state issued or FEIN or State Driver ID, or Foreign Diplomat number. May still be valid if blank, but ID number entered in Part 5 below.
  4. Name of seller.
  5. At least one selection in Part 4: type of business.
  6. At least one selection in Part 5: reason for exemption. ID number must be entered in this section or in item 3 above.
  7. Signature of purchaser.

Resale Certificate Form ST-3 

  • Replaced in Aug. 2006 by SST form
  • State prefers the new SST form, but the older form is still acceptable

 

Required fields:

  1. Purchasers NJ certificate of authority number.
  2. Name of seller.
  3. Date.
  4. Part (2) purchaser business description and products sold.
  5. Part (3) merchandise purchased.
  6. Part (4) a, b, or c box selected.
  7. Part (5) a or b box selected.
  8. Name of purchaser, including street address, city and state.
  9. Signature of purchaser.

Exemption Certificate Form ST-4

  • Replaced in Aug 2006 by SST form
  • State prefers the new SST form, but the older form is still acceptable. This is an exemption certificate for businesses not resale (ST-3) and not exempt organizations (ST-5).

 

Required fields:

  1. Purchasers NJ certificate of authority number.
  2. Name of seller.
  3. Date.
  4. Reason for exemption, may just fill in the NJSA 54:32B number.
  5. Name of purchaser, including street address, city and state.
  6. Signature of purchaser.

Contractor Exemption Certificate Form ST-13

  • Replaced in Aug 2006 by SST form
  • This form may be utilized by a contractor for work performed at an exempt site. The ST-13 must be accompanied by the ST-5 form or else work order or purchase order from a federal, state or local government.

 

Required fields:

  1. Purchasers NJ certificate of authority number.
  2. Name of seller.
  3. Date.
  4. Reason for exemption, may just fill in the NJSA 54:32B number.
  5. Name of purchaser, including street address, city and state.
  6. Signature of purchaser.

Resale Certificate Form ST-3NR

  • Replaced in Aug 2006 by SST form
  • This form is to be used only by out of state vendors not registered in NJ

 

Required fields:

  1. Name of seller.
  2. Name of purchaser.
  3. Out of state registration number for resale.
  4. Type of business and description of items sold.
  5. Signature of purchaser and date.

Notes:

  • Agencies of the federal government and the United Nations as well as the state of New Jersey and its political subdivisions are exempt from paying sales tax provided the agency making the purchase supplies the seller with a copy of a valid purchase order or contract signed by an authorized official.  No exemption form is required, but the state suggests that the SST form be completed to document the sale. Also, state issued letters documenting the buyer as one of these types will be sufficient as an exemption certificate.
  • Exempt organizations are normally issued a letter from the state, the ST-5 form. They may then insert the sellers name and description of what they are purchasing to complete the document as a valid exemption certificate.
  • A copy of the state issued letter showing the registration of the business is not a valid exemption certificate.
