Connecticut Form Guide

Sales & Use Tax Resale Certificate Form Cert-100

Required fields for the front page:

  1. Name of seller.
  2. Purchaser’s business name and address.
  3. Check at least one box of Wholesaler, Retailer, Etc.
  4. List CT state and state registration number.
  5. Description of merchandise purchased.
  6. Purchaser signature.
  7. Date signed.

Notes:

  • The Connecticut resale form will sometimes include cities and other localities. If other states are included on this form, and they are allowed by that state to be utilized on a multi-Jurisdiction form they will also be deemed valid and recorded as a resale certificate.

Sales & Use Tax Exemption Certificate Form Cert-100

Required fields for the back page:

  1. Purchaser’s business name and address.
  2. CT registration number.
  3. Name of seller.
  4. Check Blanket or Single Use Certificate.
  5. Check at least one of Materials, Tools, or Fuel.
  6. Description of merchandise purchased.
  7. Purchaser signature.
  8. Date signed.

Notes:

  • CT statute on exemption certificates is 12-426-1. Exemption are covered in 12-412.
  • Resale certificates should be renewed every three years. Charitable organization exemption certificates should be renewed every year.
