Wisconsin Form Guide

Streamlined Sales Tax Form 

  • Rev. 3/06-2

 

Required fields:

  1. State filled in as WI.
  2. Name and address of the buyer.
  3. Purchaser’s tax ID number with state issued or FEIN or State Driver ID, or Foreign Diplomat number. May still be valid if blank, but ID number entered in Part 5 below.
  4. Name of seller.
  5. At least one selection in Part 4: type of business.
  6. At least one selection in Part 5: reason for exemption. ID number must be entered in this section or in item 3 above.
  7. Signature of purchaser.

Sales and Use Tax Exemption Certificate S-211 (R-05)

Required fields:

  1. Name of purchaser, including street address, city and state.
  2. Description of property.
  3. Name of seller.
  4. Either/Or:
    1. Resale box with state issued number.
    2. One or more exempt reasons marked.
  5. Signature of purchaser and date.

Notes:

  • As of 10/1/09 WI is now an SST state and will accept the standard SST form. A certificate is not required by law and sales can be exempt if all the data elements are collected.
  • Sales to federal government, state government municipalities or schools can be documented by (a) exemption certificate, (b) purchase order identifying goods for government unit or (c) recording Certificate of Exempt status (CES) number on the invoice.
  • Form S-211 was issued to replace form s-016, s-017, s-205, s-206, s-207, s-207m and s-209. Older forms are still valid as proof of exempt sales if properly completed and taken in good faith.
