Streamlined Sales Tax Form

Required fields:

  1. State filled in as WV.
  2. Name of purchaser, including street address, city and state.
  3. Purchaser’s tax ID number with state issued or FEIN or State Driver ID, or Foreign Diplomat number. May still be valid if blank, but ID number entered in Part 5 below.
  4. Name of seller.
  5. At least one selection in Part 4: type of business.
  6. At least one selection in Part 5 Reason for exemption. ID number must be entered in this section or in item 3 above. WV Does not accept C or K as valid exemption reasons on the SST form.
  7. Signature of purchaser.

CST-280 Exemption Certificate

Required fields:

  1. Name of seller.
  2. Date.
  3. Must mark Blanket or Single purchase certificate.
  4. Must include their state issued business registration number.
  5. Select the reason for exemption by checking one of the boxes.
  6. Buyer name.
  7. Signature.


  • Exemption and sales tax rules are established in Code 11-15 and 11-15B for SST regulations. WV started allowing the SST form in 2004 and expiration dates were extended to allow documents to never expire.
  • WV still does not accept the MTC form.
  • Under new SST regulations governments, churches, school and other exempt organizations must complete the SST exemption form or should be charged sales tax. West Virginia does not accept reason C- Tribal Government or K Direct Mail as valid exemption reasons
