Streamlined Sales Tax Form  Rev. 3/06-2

  • Never expires.

 

Required fields:

  1. State filled in as WA.
  2. Name and address of the buyer.
  3. Purchaser’s tax ID number with state issued or FEIN or State Driver ID, or Foreign Diplomat number. May still be valid if blank, but ID number entered in Part 5 below.
  4. Name of seller.
  5. At least one selection in Part 4: type of business.
  6. At least one selection in Part 5: reason for exemption. ID number must be entered in this section or in item 3 above.
  7. Signature of purchaser.

Notes:
Effective July 1, 2008, Washington State revised it’s regulations to come in-line with SST. RCW 82.04.470 also states that if the data elements are delivered in a form other than paper (such as email) then Name of Seller (4) and Signature (7) of the above list are not required. Resale certificate documents that were made available by the state in 2009 had the statement (Valid through December 31, 2009) printed at the top of each form.

  • Per regulations these forms are no longer allowed after 12/31/09
  • Older forms on file must be replaced by the SST, MTC or Resale Permit

 

Required fields:

  1. Name of seller.
  2. Name of purchaser, including street address, city and state.
  3. Purchaser’s tax ID number.
  4. Description of items purchased.
  5. At least one selection for reason for exemption.
  6. Signature of purchaser.
  7. Effective date and expiration date.

Notes:
If the effective date and expiration date are not filled in, then the fax date, signed date or received date will become the effective date. The expiration date should be calculated as 4 years after the effective date. As of 1/1/09 if the expiration date on these older forms is blank, it will still be accepted as valid. Under the new regulations the resale certificates do not require an expiration date.

Manufacturer’s Sale and Use Tax Exemption Certificate Rev 27

Required fields:

  1. Purchaser’s registration number.
  2. Name of purchaser, including street address, city and state.
  3. Name of seller.
  4. Signature of purchaser.

Notes:

  • Senate Bill 6173 no longer allows resale certificates. Due to SST requirements they will still allow the use of the SST and MTC form, but the state issued exemption certificate is no longer accepted. The state issued resale license or reseller permit is now the accepted form.
  • It is recommended that resale certificates be renewed every four years and Contractor certificates be renewed every year.
  • Federal government may purchase exempt with just a letter stating they are buying on behalf of the federal government. State, local, school, churches and other exempt organizations should utilize the SST form to show they are exempt.
  • Tax ID numbers can be validated online.

    • Forms began being used in 2010 and expiration dates may be 12 months or up to 4 years, depending on the nature and filing history of the company.
