Resale Certificate Form R-1042

  • Retired 10-31-09
  • This form is only valid in 2008 and 2009. All must expire on 10/31/09 when it was replaced by the State issued R-1064 form.

 

Required fields:

  1. Name of purchaser, including street address, city and state.
  2. Sales tax number, must be 10 digits and does not need to include a W.
  3. Purchasers type of business.
  4. Description of items to be purchased.
  5. Name of seller.
  6. Signature of purchaser.
  7. Date.

 

Resale Certificate Form LGST-9-A / R-1079

  • Retired 1-1-09

  • This form is only valid for periods prior to 2009, when it was replaced by form R-1042.

 

Required fields:

  1. Date.
  2. Name of purchaser, including street address, city and state.
  3. Sales Tax number, can be an out of state number.
  4. Name of seller.
  5. Signature of purchaser.

Blanket Exemption Certificate Form LGST-9 / R-1028

  • Retired 1-1-09
  • For purchases by registered wholesalers – state issued
  • This form is only valid for periods prior to 2009, when it was replaced by form R-1042.

 

Required fields:

  1. Date, in many cases both effective and expiration date are on the certificate and will be recorded.
  2. Name of seller .
  3. State registration number, must include “W”  Preprinted on form by state.
  4. Description of items purchased.
  5. Name of purchaser, including street address, city and state. Pre-printed on the form by the State.
  6. Signature of purchaser.

 

Notes:

  • Per RIB 09-15 – no certificates are acceptable after 10/31/09 for resale. “Dealers” may utilize the state issued license. The term dealer includes every person who manufactures property for resale. Exemption certificates are still available for use in purchasing manufacturing equipment or farming supplies, but they must use the specific exemption form. No general forms are allowed.
  • LA does not accept the MTC or SST exemption form.
  • LA revised the regulations and moved to the new 1042 form in October 2008. Previously, if a company is registered in the state of Louisiana, then they should provide the R-1028 state issued form. If they are an out of state registered company, then they should issue the R-1079 form.
  • Per RS 47:301(8)  The state of Louisiana, it’s parishes, its municipalities and any other agencies are excluded from sales tax. No certificate is needed. Also recognized colleges and universities are exempt. They recommend the use of form R-1056 to identify themselves as state agencies.
  • LA state exemption forms should be entered and applied to the state/county/city/district levels unless they specify that they should only be applied to the state exemption level. Each parish and city has the ability to generate their own exemption certificates. Each will have specific rules that must be applied, such as only valid for one year, or applies to city level only. Unless it is stated on the form, these exemptions will not expire and must be fully completed to be applied.
  • As of 11/1/09 resale exemption certificates expire by the date on the state issued form. Exemption certificates should be renewed every three years and charitable exemptions should be renewed every five years.
  • Resale certificate numbers can be validated online.
